Job description
This position provides social work services to patients and families within the hospital setting. Serve as an
integral member of the interdisciplinary healthcare team. Social Work Care Manager will work to identify
potential social work needs/supports in collaboration with the inpatient and ambulatory care teams, the patient
and support system. The Social Work Care Manager will consistently and thoughtfully apply social work values.

Principal Accountabilities:
Care Coordination -
A. Patients requiring social work intervention will be provided a comprehensive assessment including; patient’s/family’s psychosocial risk factors and transitional/discharge needs through evaluation of
prior functioning levels, appropriateness, adequacy of support systems, reaction to illness, ability to cope.
B. Documents and collaborates with Case Manager, physician and multidisciplinary team regarding findings.
C. Actively participates on interdisciplinary team to develop and implement care plans during hospitalization.
D. Establishes and implements social work care plans to ensure safety, continuity of care, appropriate care
coordination and support post hospitalization, working with community providers and resources.
E. Knowledge of community resources, healthcare funding, county, state, and federal regulations in making
appropriate and safe plan.
F. Documents work clearly, concisely and consistently in a timely manner.
G. Actively participates in clinical performance improvement activities as assigned
H. Participates in Outlier Rounds with Physician Advisor as needed.
Relationship/Teambuilding - A. Maintains excellent working relationships and works collaboratively with
departmental colleagues
B. Functions independently as a part of an interdisciplinary team, working effectively within departmental
expectations as well as patients, support system/medical team to facilitate quality, efficient patient care.
C. Attends interdisciplinary rounds, participates in progression of care discussions, barriers,and discharge plan.
D. Enhances departmental relationships and activities through active participation in staff meetings and
proactively identifying problems and solutions in the work environment. Supports other team members in
achieving patient centered goals.
E. Participates in and contributes to appropriate departmental and/or organizational goals.
Personal Development -A. Remains current in knowledge and skills of social work practice, and best practices.
B. Demonstrates responsiveness/appreciation of feedback/recommendations for growth/development.
C. Completes mandatory education/organization requirements in a timely manner and actively demonstrates
self-directed learning and continuing education to enhance professional development.
D. Willingly participates in committees, task forces, projects, PI teams. Performs other duties as assigned.

Bachelor's degree in Social Work and 1-3 years experience required. Master's degree and 3+ years experiene strongly preferred. Knowledge of the values, principles and methodologies of social work. Ability to integrate social and medical data to make psychosocial assessments and implement social work treatment plans. Ability to organize and prioritize multiple assignments within workload. Ability to deal with change and ambiguity
